What a pressure injury is

A pressure injury, also called a pressure ulcer or bedsore, is localized damage to the skin and underlying tissue. It usually forms over a bony area such as the heel, hip, tailbone, or back, where sustained pressure reduces blood flow. Friction, shear, moisture, and limited mobility all make them more likely.

Pressure injuries are commonly described in stages, from early skin redness that does not blanch (Stage 1) through partial and full-thickness tissue loss (Stages 2 to 4), plus deep tissue injury and unstageable wounds where the wound bed is covered. Accurate staging matters, because it drives the entire treatment plan.
